Health professionals within the Community Learning Disabilities Team (CLDT) may refer directly to the department.
Referrals from other sources are made via the CLDT through the referral Integrated Care Pathway (ICP). Referral is subsequently made to the department following screening by a Senior Health Care Coordinator.
Health professionals working at Kirklands Hospital or within the Community Learning Disabilities Team (CLDT) may refer directly to the department. If a client is already open to the CLDT an Inter team referral can be made for a functional assessment.
Referrals to the department are made for a variety of reasons relating to difficulties with function in the broad areas of self-maintenance, productivity and leisure, where the person has an associated learning disability and where the learning disability places limitations on their level of functioning, resulting in their inability to access generic services effectively.
Referral to Occupational Therapy should reflect the need to promote health, prevent disability and to restore, maintain or improve function, well being and social integration while engaging in activities relating to:
- Self Maintenance (self care, eating & drinking process and equipment, domestic skills, access, road safety, transport and social skills for community living)
- Productivity (analysis of task, environmental and system demands on work skills, education, training, creative activities)
- Leisure (access and suitability of community based activities, adaptation within and access to sport, advice about and adaptation of suitable recreation activities)
