The Braveheart project’s objectives are:
- To promote awareness of and identify the risks of coronary heart disease, diabetes, hypertension, coronary vascular angina.
- To identify lifestyles and the ability to change.
- To encourage evidence-based practice for primary prevention of heart conditions and implementation of appropriate standards.
- To provide education to equip people with identified risks with the knowledge, attitude and motivation to effectively change or modify their lifestyle to maximize their health.
- To promote the integration of secondary coronary heart disease clients amongst healthcare professionals enabling the formation of secondary coronary heart disease registers.
- To produce evidence-based link guidelines for primary prevention of chronic vascular angina, coronary heart disease, diabetes, early detection of hypertension and improve the overall health of Lanarkshire.
