New Horizons - A Plan for Modernising Primary Care Health Services

NHS Lanarkshire has developed a plan to outline a clear path for the development of primary and community services in Lanarkshire over the next ten years.

New Horizons – A Plan for Modernising Primary Health Care Services in Lanarkshire is still in draft form and will go to the 25 June meeting of NHS Lanarkshire Board for consideration.

The draft plan sets out the case for change, identifies four cornerstones of care, and describes the desired outcomes and how they will be delivered.

The four cornerstones of care set out in the plan are:

• Health Improvement
• Unscheduled Care
• Care for People with Long-Term Conditions
• Extended Primary Care
